Everlast Digital Forms is a custom web application at dashboard.everlastdigitalforms.com that gives Everlast Wellness teams a centralized portal for digital form submission, review, and operational workflows. I built the authenticated login gateway and dashboard experience with role-aware access — designed so clinical and administrative staff can capture reliable data without paper processes or disconnected tools.

The app extends the Everlast internal-systems ecosystem alongside ERP, intranet, and payments — demonstrating full-stack delivery of healthcare operations software from auth through production deployment.
